Once again, I apologize to everyone who tried to fill it out and had it get erased or they got an error message. In the end, it was for the best because now the survey has been totally re-written more questions, more options, clearer language and a much better interface. You can also start the survey and finish it off later on if need more time.
If you were one of the few people who did fill out the last one, I kindly ask you come back and refill the survey as there are new questions and all the old data was lost. I'm really sorry and I understand your frustration and hope you will take the time to fill out the more complete survey.
